All ACHS Patients have access to a provider 24/7 outside of normal business hours by calling 603.444.2464
Do not use this website for emergencies. If you are experiencing a life threatening or urgent problem, please call 911 immediately.
The patient portal will help you securely communicate with your provider, keep your information up to date and give you access to a summary of your medical records 24 hours a day. Once you register, you’ll be able to:
- View your medical records
- Complete pre-appointment forms
- Update your personal information
- Request medication refills
- Request non-urgent appointments
- Pay your bill
- Email your provider
- Access lab results
New to the Portal? If this is your first time accessing the ACHS patient portal, you’ll need to set up an account. Get started by requesting your secure PIN number by calling 603.444.2464. The secure PIN will be emailed to you and will expire in 30 days. To ensure privacy, we do not give PIN numbers over the phone. Each patient is required to have a unique email address for the portal. Spouses, parents and legal guardians must create separate accounts. Once you have your username and password, follow the instructions (link to Portal reg help PDF) to active your account.
