Theresa O’Doherty is a Family Nurse Practitioner and is licensed as an Advanced Registered Nurse Practitioner (APRN) in CT, MA, NH, NY, and RI. Theresa has more than ten (10) years of experience as an APRN and has worked in FQHC’s, VA specializing in family medicine, urgent care, substance abuse, complex geriatric care and surgical oncology. Theresa has received specialized training in hospice/palliative care, Alzheimer’s research, dermatology and woman’s health. She is a retired Army Non-Commissioned officer and decorated combat veteran. Theresa holds a A.S. in science from Mount Wachusett Community College, B.A. in English from Boston College, B.S. and M.S. in Nursing from Regis College. She is specialty trained in alternative medicine modalities, hospice/palliative care, Alzheimer’s research, dermatology and woman’s health. She is a retired Army Non-Commissioned officer and decorated combat veteran. Theresa has a passion for advancing the delivery of healthcare during changing times, while providing provider support and improving access to high quality modernized care for patients. Her hobbies include spending time with her children, family and friends, exercising, gardening, cooking and traveling the world.

Liz is a Family Nurse Practitioner and is licensed as an Advanced Registered Nurse Practitioner (APRN) in CT, MA, NH, NY, and RI. Liz has more than 12 years of experience as an APRN and has worked in family medicine, urgent care, substance abuse, vascular surgery and complex geriatric care settings. She is a decorated Army combat veteran. Liz holds a B.A. in English and M.S. in Administrative Studies from Boston College, a B.S. and M.S. in Nursing from Regis College. She has a passion for geriatrics with a focus on prevention, complex chronic disease management and quality of life for providers and patients. She is a dedicated provider with extensive knowledge and experience in complex care management and uses skills for improvement of care in all levels of the healthcare delivery system. Her hobbies include spending time with her family and pets, hiking, exercising, camping, gardening, canning and baking.

Amy is a compassionate and knowledgeable healthcare provider who is committed to helping people and families stay healthy in both body and mind. With advanced education and specialized training, she is able to identify and treat a wide range of health issues. Amy provides personalized care to make sure everyone gets the help they need.

Setting out from the heart of south Texas, we are incredibly excited to introduce to you, Dr. Andres Aguilera-Fish—a first-generation Mexican-American who’s journeyed far and wide. Since 2005, with just a brief break, he’s poured his time and passion into his higher education right here in New Hampshire. Freshly back from New York, where he completed his Family Medicine Residency, Dr. Aguilera-Fish is thrilled to call the North Country his new home. During his residency, he dedicated himself to working with the transgender community and honed expertise in mental health and addiction medicine. In the North Country, Dr. Aguilera-Fish eagerly joins this incredible community and can’t wait for the outdoor adventures that lie ahead.

I like to help people navigate difficult times, as sometimes just another person being present can make all the difference. ACHS provides that place and connection.

I wanted to work in family medicine to care for patients throughout their lives and to provide holistic family-centered care to those in rural underserved areas of need. I truly enjoy working with families and patients of all ages to help them achieve the best quality of life and health possible.  As a community health center, ACHS is an ideal place to address health care disparities so all patient needs can be fully met.

Patient care needs to be holistic, taking into account the whole person and how all aspects of their life may be affecting their health. I’m excited to join an integrated team that promotes detection, prevention, and treatment of mental disorders. With readily available behavioral health providers patients can easily access the care they may otherwise not have been able to access. Regular communication between providers can ensure comprehensive care delivery and improve patient outcomes.

Providing psychiatric care in the primary care setting offers the best opportunity to reach people who may have previously not been able to or hesitant to try. Ammonoosuc’s program highlights how the health of the mind and body are indivisible—equally important for all ages.

Delivering psychiatric care across the continuum within the primary care model improves patient outcomes and reduces the stigma of mental illness.

As a family physician involved in the health care of this community for many years, I understand more fully the prevalent medical and social needs of the community and am enthusiastic to return to the community health center setting and work with the team at ACHS and their patient-centered approach to care.

I enjoy caring for patients as they grow and change–whether it be from teens to adults or from middle to older age. It is a privilege to care for families as they experience the joy of a new baby or face the challenge of illness. I am happy to provide prenatal care with patients delivering with obstetricians at Littleton Regional Hospital or DHMC. I also perform procedures such as IUD insertion, colposcopy, skin procedures, and some joint injections.

The best healthcare is that which recognizes all the facets of a person, recognizes their insights and strengths then works as a team within a community to promote health and prevent illness.

Everyone experiences illness in his/her own way. Caring for the patient, as well as understanding how to treat the disease, is key to great healthcare. Preventing chronic medical issues requires a partnership between the patient and the provider.

If you come to see us with a problem, the chances are very good that you’ll leave with either a diagnosis and plan of treatment, or a clear plan for how to get there. A family practice physician’s job is to keep you well from the beginning through preventive care. When problems become more complicated, we can coordinate with specialty providers to continue to get you the health care you need. There are times when traditional medicine does not offer a cure, and just managing symptoms (or even just being present) is the best that we can offer.

Working at ACHS gives me three very powerful tools to keep my patients as healthy as possible:

• Involvement in the community. My patients are my friends and neighbors – there is no more powerful motivator to go the extra mile, do the extra research, make one more phone call to figure things out.
• Continuity. My patients come back again and again, for years, even decades. Many go back to birth and even before that with prenatal care. The perspective gained over many visits over many years often yields greater insight into a person’s health than any high-tech imaging modality.
• Knowledge of the community. The staff at ACHS knows how to connect their patients with the right resources to achieve successful end results. A brilliant diagnosis is great, but sometimes a brilliant phone call that connects a patient with a service they need is even better.

My whole life, I’ve wanted to care for people. Working in a community health center offers an ideal way to truly connect with patients. I enjoy listening and aiding my patients so they may be advocates for their own health.
