Nicole Fischler, a Family Nurse Practitioner with over 18 years of experience, graduated from Simmons College in 2003 with a deep dedication to holistic healthcare and community well-being. Beginning her career in Boston, she nurtured her love for community health centers. Transitioning to ACHS, she devoted 15 impactful years to patient care and community betterment. Nicole’s journey led her away briefly to be closer to family, but the pull of serving her local community was strong. Now back, her mission continues: Guiding families through generations, watching kids grow into adults, and aiding graceful aging are her joys. Healthcare, to her, is a privilege tied to each unique journey.
Professionally, Nicole focuses on chronic diseases, weight wellness, addiction care, LGBTQ+ support, and de-prescribing. ACHS’s values align with her goal: a collaborative healthcare model erasing access barriers. Beyond healthcare, Nicole finds balance in the White Mountains’ embrace. Gardening, biking, hiking, and shared meals rejuvenate her spirit.

Setting out from the heart of south Texas, we are incredibly excited to introduce to you, Dr. Andres Aguilera-Fish—a first-generation Mexican-American who’s journeyed far and wide. Since 2005, with just a brief break, he’s poured his time and passion into his higher education right here in New Hampshire. Freshly back from New York, where he completed his Family Medicine Residency, Dr. Aguilera-Fish is thrilled to call the North Country his new home. During his residency, he enhanced his expertise in diagnosing, treating, and supporting individuals with mental health conditions and substance use disorders through clinical training, direct patient care, and mentorship from experienced professionals.
I like to help people navigate difficult times, as sometimes just another person being present can make all the difference. ACHS provides that place and connection.

I enjoy helping to provide access to healthcare for all people, especially in underserved and rural areas.
I wanted to work in family medicine to care for patients throughout their lives and to provide holistic family-centered care to those in rural underserved areas of need. I truly enjoy working with families and patients of all ages to help them achieve the best quality of life and health possible. As a community health center, ACHS is an ideal place to address health care disparities so all patient needs can be fully met.
I believe firmly that a holistic approach to healthcare that addresses both the mind and the body leads to improved health and a deeper sense of wellness. I joined ACHS for the opportunity to be a part of an agency that works to break the stigma around all types of illness and respects the patient’s rights to self-determination.
I’m excited to join an integrated team that promotes detection, prevention, and treatment of mental disorders. With readily available behavioral health providers patients can easily access the care they may otherwise not have been able to access. Regular communication between providers can ensure comprehensive care delivery and improve patient outcomes.
Delivering psychiatric care across the continuum within the primary care model improves patient outcomes and reduces the stigma of mental illness.
