A long-time Littleton resident, Linne’ Humbargar has returned to the North Country full-time after spending over a decade delivering high-quality care across the U.S. and Europe. As a medical traveler, she worked in diverse clinical settings, including the Department of Defense, Indian Health Services, inner-city clinics, and other Federally Qualified Health Centers (FQHCs). With 25 years of experience in healthcare—including 15 as a nationally board-certified Physician Assistant—Linne’ brings a wealth of expertise in both Primary and Urgent Care.
Beyond medicine, she enjoys the North Country lifestyle: hiking, cycling, painting, and spending time outdoors with her pets. Her return to ACHS is a homecoming rooted in her love for the region and its people.

My approach to patient care centers around education and empowerment. I believe in giving patients the knowledge and tools they need to take charge of their health—whether it’s understanding medications, preventing disease, managing chronic conditions, or making lifestyle changes that support long-term well-being.
Outside of work, I stay active through bicycling, skiing, paddleboarding, and climbing—activities that keep me grounded and inspired to support others in leading healthy, active lives.

My therapeutic approach is eclectic and adaptable—I don’t believe in a one-size-fits-all model. I strive to create a supportive, safe space where individuals can explore their needs, define their goals, and discover what truly motivates them. Together, we work to find personalized strategies that encourage meaningful and lasting change.
Outside of work, I’m passionate about travel, specialty coffee drinks, and experimenting in the kitchen. I also have an ever-growing collection of craft supplies—definitely more than any one person reasonably needs!

As a family physician, Dr. Smith believes in the vital role of both effective disease management and high-quality preventive care to reduce suffering and promote long-term health. She sees it as a responsibility to deliver this care with empathy—not only toward patients, but also colleagues and herself.
A long-time resident of Littleton, Dr. Smith enjoys gardening, bicycling, snowshoeing, and practicing meditation outside of her medical work.

Nicole Fischler, a Family Nurse Practitioner with over 18 years of experience, graduated from Simmons College in 2003 with a deep dedication to holistic healthcare and community well-being. Beginning her career in Boston, she nurtured her love for community health centers. Transitioning to ACHS, she devoted 15 impactful years to patient care and community betterment. Nicole’s journey led her away briefly to be closer to family, but the pull of serving her local community was strong. Now back, her mission continues: Guiding families through generations, watching kids grow into adults, and aiding graceful aging are her joys. Healthcare, to her, is a privilege tied to each unique journey.
Professionally, Nicole focuses on chronic diseases, weight wellness, addiction care, and de-prescribing. ACHS’s values align with her goal: a collaborative healthcare model erasing access barriers. Beyond healthcare, Nicole finds balance in the White Mountains’ embrace. Gardening, biking, hiking, and shared meals rejuvenate her spirit.
New patients of all ages are warmly welcomed by Nicole. Her return ensures compassionate care, holistic guidance, and a true commitment to your well-being.

Setting out from the heart of south Texas, we are incredibly excited to introduce to you, Dr. Andres Aguilera-Fish—a first-generation Mexican-American who’s journeyed far and wide. Since 2005, with just a brief break, he’s poured his time and passion into his higher education right here in New Hampshire. Freshly back from New York, where he completed his Family Medicine Residency, Dr. Aguilera-Fish is thrilled to call the North Country his new home. During his residency, he enhanced his expertise in diagnosing, treating, and supporting individuals with mental health conditions and substance use disorders through clinical training, direct patient care, and mentorship from experienced professionals.

ACHS, a non-profit Federally Qualified Community Health Center established in 1975, operates in five sites in Northern New Hampshire. They provide affordable and high-quality healthcare to over 10,000 patients in more than 26 towns, offering integrated health services to families, including Medical, and Behavioral health. ACHS serves everyone and does not deny services based on inability to pay. This health center receives HHS funding and has Federal Public Health Services (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
