We are excited to announce that ACHS has been approved to administer the COVID19 Vaccine as part of the US Health Resources & Services Administration (HRSA) Health Equity Vaccine Program. These clinics begin on March 6, 2021, at our ACHS-Woodsville location and are BY APPOINTMENT ONLY, and based solely oneligibility as determined by the NH Department of Health & Human Services (NH DHHS). At this time, our vaccine supply is very limited. As our supply of vaccines increases, we expect to be increasing availability and clinics.
Registration for the vaccine and appointment scheduling is through the VAMS website. There is NO COST for the COVID19 vaccine and you do not need to be a patient to get the vaccine at ACHS. However, you do need to qualify and register with the state of NH first.
HOW TO GET YOUR VACCINE AT ACHS:
Step 1: Check YOUR Eligibility (currently Phase 1b qualifying individuals) Currently available to NH residents only
Step 2: If eligible, register with the VAMS website. Don’t have internet access? Call 2-1-1
Step 3: Select an ACHS location on the VAMS website. The ACHS location will show HRSA-Ammonoosuc
Step 4: Select an appt time and location for your first vaccine shot
Step 5: Come to your appointment with your NH photo ID and wear a short sleeve shirt layer
Please complete your medical information forms (sent by email from VAMS) prior to your arrival and bring them and your NH photo ID to your appointment. Woodsville COVID19 Vaccine Clinics are held inside the ACHS building. Follow signs for parking and entry.
FOR OUR VT PATIENTS: Unfortunately, state guidelines have limited vaccines to NH residents only. Contact the Vermont Department of Health about how and where to get the COVID vaccine.
